Simple midi + audio interface

Does anyone make a *simple* USB audio interface that Also has MIDI on
it?  Here are my requirements:

1.  MIDI IO
2.  USB
3.  One stereo input
4.  One stereo output
5.  Preferably 1/4 inch jacks


Thats it.  Nothing complicated, however it seems everything out there
either has way to much, or is missing something else.

I am looking for a way to get my netbook into my jam rig.  Now that my
EMX1 is finally completely dead, I want to use it running FL Studio or
Renoise for doing beats with the modular.
